All employees of the University are covered by workers compensation insurance in the event of a work-related injury or illness.  

Employer's First Report of Injury or Illness Form

Coverage Highlights:
  • Workers compensation insurance pays the cost of medical expenses incurred as a result of the work-related illness/injury.
  • After an employee has been off work as a result of a work-related illness/injury for three days, workers compensation pays 2/3 of income replacement.
  • Report the injury or illness to your supervisor and to the Absence Management Team Human Resources.
In the event of a Work-related Injury/Illness:
  • Report the injury or illness to your supervisor and to Human Resources
  • Seek appropriate medical attention.
    • For a life-threatening injury or illness, call 911 or go to the nearest emergency medical provider.

  • Do not disturb the accident site until it has been investigated and released. Tools, equipment, materials or other evidence that might pertain to the cause of the accident must not be removed or destroyed until authorized by the Labor Commission or one of its compliance officers.
  • Complete the Employer's First Report of Injury or Illness Form and submit it to the Absence Management Team within 24 hours by emailing AbsenceManagement@utah.edu. 
  • If the injury or illness is disabling or serious, including overnight hospitalization, broken bones, loss of limb, loss of consciousness, deep cuts, electric shock, sight impairment, or a fatality, the employee or supervisor must contact the University’s  Occupational and Environmental Health and Safety  (OEHS) office immediately.  OEHS must notify the Utah Occupational Safety and Health Division within 8 hours of the incident.
