LIFE INSURANCE
The University of Utah offers group life insurance for benefit-eligible employees. The University provides basic coverage equal to the employee's annual salary up to $25,000. Employees can purchase additional coverage for themselves, their spouse or domestic partner, and children. Travel Assistance is also included for eligible employees.
University Provided Coverage
Coverage provided to all benefit-eligible employees
- Basic Employee Life: An amount equal to your annual salary, up to a maximum of $25,000
Elective Coverage
- Employee:
- Basic Employee Life: An amount equal to the University-provided coverage
- Employee Additional Life: Enroll up to $500,000 without proof of good health, see below
- To increase up to 5 times annual salary (Maximum $1,000,000) complete the Medical History Statement at any time
- Family:
- Basic Family Life: $2,000 for spouse/domestic partner or child(ren)
- Spouse/Domestic Partner Additional Life: enroll up to $50,000 without providing proof of good health, see below
- To increase up to $250,000 complete the Medical History Statement at any time
- Child Additional Life: Dependent children under age 26 in the amount of $5,000 or $10,000

Newly benefit-eligible employees will automatically be enrolled in the following coverage:
- Basic Employee Life: An amount equal to your annual salary, up to a maximum of $25,000
Elective Coverage
Newly benefit-eligible employees can enroll in elective coverage through UBenefits with 30 days eligibility
- Employee:
- Basic Employee Life: An amount equal to the University-provided coverage
- Employee Additional Life: Enroll up to $500,000 without proof of good health, see below
- To increase up to 5 times annual salary (Maximum $1,000,000) complete the Medical History Statement at any time
- Family:
- Basic Family Life: $2,000 for spouse/domestic partner or child(ren)
- Spouse/Domestic Partner Additional Life: enroll up to $50,000 without providing proof of good health, see below

If you experience a qualifying life event (e.g., marriage, birth/adoption of a child), you have 30 days to update your coverage on UBenefits:
Marriage/New Domestic Partnership
- Employee
- Basic Employee Life: An amount equal to the University-provided coverage
- Family
- Basic Family Life: $2,000 for spouse/domestic partner or child(ren)
- Spouse/Domestic Partner Additional Life: enroll up to $50,000 without providing proof of good health, see below.
- To increase up to $250,000 complete the Medical History Statement at any time
Birth/Adoption/Foster Care/Legal Guardianship
- Employee:
- Basic Employee Life: An amount equal to the University-provided coverage
- Family:
- Basic Family Life: $2,000 for spouse/domestic partner or child(ren)
- Child Additional Life: Dependent children under age 26 in the amount of $5,000 or $10,000

The following change can be made during Open Enrollment that runs annually from May 1 to May 31. All elections made during this period will take effect on July 1.
Employee Life
- Basic Employee Life
- Increase Employee Additional Life by $20,000, up to of $500,000 maximum
- To increase by more than $20,000, complete the Medical History Statement at any time
Family Life
- Basic Family Life
- Increase Spouse/Domestic Partner Additional Life, by up to $20,000, up to $50,000 maximum
- To increase by more than $20,000, complete the Medical History Statement at any time
- Add or Increase Child Additional Life for dependent children under age 26 in the amount of $5,000 or $10,000
Employee Life Insurance
- Primary Beneficiary: At least one per policy
- Contingent Beneficiary: At least one per policy
Family Life Insurance
- Primary Beneficiary: Employee (automatically assigned)
- Secondary Beneficiary: At least one per policy
Update or Change Your Beneficiaries
To update or change a your beneficiaries at anytime through UBenefits, follow the steps below:
- Log in to CIS and go to the Benefits Information tab
- Select UBenefits to access your benefits dashboard
- Click on Change Your Benefits (this should be the large box on the page)
- Scroll past the Life Events section (e.g., Add a Child, Add a Spouse/Partner) until you reach the Other Events section.
- Locate Update Beneficiaries and select the Red Start Button to proceed
- On the Update Beneficiaries page, click the next Red Start Button to enter the event
- Click Edit Beneficiaries to update your information
- After making changes, select Save and Continue at the bottom of the page
- You can copy your updates to other plan elections or continue making additional changes
- Once all updates are complete, click Continue and review your changes on the Checkout Page
- If everything is correct, click Submit to finalize your updates
- You will receive a confirmation email once the changes are submitted
Travel Assistance is included in the University-provided life insurance coverage for all benefit-eligible employees
If you are leaving the University and would like to continue your life insurance coverage through an individual policy, you may be eligible for portability or conversion of your current coverage. You must apply within 60 days of your last day of work.
