Annual Physical/Wellness Exam
To receive WellU credit for your annual physical exam, you must visit a health care provider of your choice and ensure the visit is billed through your health plan. This is required for your completion to be recorded.
Please Note:
- To qualify, your visit must be billed as a physical exam. Visits billed as general office visits will not meet the WellU requirement.
Tracking Your Completion:
The WellU Team does not have access to employee medical records, including MyChart or EPIC. Completion data is reported directly by the Health Plan Administrator and RedMed on a monthly basis. The WellU Team is not able to manually enter completions.
If Your Completion Is Missing:
If your physical exam does not appear on your WellU Participation Tracker after 90 days, please contact WellU@utah.edu for assistance.
Prenatal Wellness Exams:
Prenatal exams do qualify as a physical; however, because prenatal visits are billed differently, they require additional documentation. Please have your provider complete the Prenatal Physical Certification Form and submit it to WellU@utah.edu.
Note: This form is only required for prenatal exams—not for standard annual physicals.
