    • Frame: Up to $175 at Moran Vision Center; up to $150 with other providers
    • Lenses: Up to $220 combined at Moran Vision Center; up to $160 with other providers

How to Cancel:

The vision plan is a year-long commitment. Employees can cancel during annual enrollment by logging into UBenefits Plus Corestream Portal and selecting “Make Changes” or by contacting customer service at utah.edu@corestream.com or (801) 640-9899.
